Equipment Request Form

This form can be used to request equipment.  Equipment used for events should be scheduled in coordination with the room/space reservation.

This form is a request form only. All requestors will be notified via e-mail or telephone from our office with validation the equipment you requested is available and the request can be honored.

Equipment pickup and return

All equipment can be picked up and returned at the Helpdesk.

Requests can be picked up between 8 a.m.-4:30 p.m. on the date of pickup.
Requests must be returned between 8 a.m.-4:30 p.m. on the date of return
